TO – DO LIST FOR PRACTICE ASSESSOR

• Communicate and notify trainee four weeks before the planned QIP Practice Visit

​

• The trainee is expected to send by email to you the completed QIP Level 1 form two weeks prior to the visit. Go through the forms for possible areas to explore.

​

• On the Visit Day, the trainee is to hand over a print-out of the QIP Level 1 forms 

​

• Verify from case notes and other sources e.g. staff, patients, that the particulars written are accurate

​

• Log – in and submit the Clinic Visit report via QIP Website:-
