Practice Diary: Conjoint MAFP/FRACGP/icFRACGP Examination Part 2 2024
 

Instructions to Candidates:

• Download the Practice Diary template below.

• Please follow the instructions as written in the Practice Diary.

• Please make sure that all parts are completed and that the required documents & pictures are attached as requested before submitting.

• The Boards reserve the right to reject any incomplete, inaccurate or LATE submission of the Practice Diary. A candidate with a rejected Practice Diary will not proceed with the Part 2 MAFP/FRACGP/icFRACGP Conjoint Examination.

• Please save your Practice Dairy using your full name in this format: "YOUR FULL NAME Practice Diary 2024"; for example, FLASH GORDON Practice Diary 2024.

• To submit your completed Practice Diary, please click the ‘Add File’ button and choose your file. Then fill in your email address and full name in the provided box. Finally, click ‘Send’ to submit. Online submission will automatically be closed by the given closing date, and any candidates failing to submit a completed Practice Diary by this date will be deemed as failing an attempt. Submission of hard copies of the Practice Diary will NOT be entertained.

• Please note that you need to keep a record of the Part 3 information. I.e. cases are taken and dates for easy retrieval when a practice audit is done to verify the information you have given in the Practice Diary.
